Standard Consent Forms

These documents are the result of our informed consent revision process which was launched in 2005. Per our new process, each form was sent through two levels of physician review, and also edited by Medical Mutual for defensibility, clarity, and health literacy. We believe that the end result of this process is an informed consent document that is comprehensive, accurate, and clear to all patients.

Note: These consent forms are posted in Microsoft Word format so that you may add your practice's logo to each form. However, we recommend that you do not make any additional changes to the content of the forms.

While the use of a consent form cannot prevent a claim or lawsuit from being filed against you, we believe that the proper use of these forms can help to minimize the risk. The forms should be used as an educational tool to reinforce information regarding the risks, intended benefits, possible complications, and alternatives to the procedure.
